New Franklin Center for Rehabilitation and Nursing is Hiring a Licensed Practical Nurse (LPN) Near Queens, NY

The Nursing Home is seeking an LPN to provide care for the residents of our facility. Excellent opportunity for job stability and good benefits!

Job Responsibilities:

1. Provides nursing treatment and personal care services nursing home residents within the scope of practice for LPNs, including treatment and prevention of Pressure Ulcers, Monitoring Food & Liquid Input/Output, Providing Patient Hygiene, Preparing Injections/Enemas, Taking Vital Signs, Feeding Patients,. Applying Dressings & Bandages, Catheter and Ostomy Management, Orthotic .

2. Performs and documents medication administration including medication pass, ordering, controlled substance reconciliation, refill and storage. Observes and/or reports any adverse reaction to medications or treatments.

3. Schedules diagnostic testing as ordered, reports laboratory/diagnostic test results to RN/Physician per facility policy

4. Maintains patients/residents medical and nursing records - entering vital signs, weight, fluid and nutritional intake and output.

5. Collects samples for testing and perform routine laboratory tests on samples as ordered.

6. Monitors IV therapy, performs prescribed treatments and procedures and dispenses medications as per order and facilities policies.

7. Investigates, reports and records all accidents of patients and staff to nursing supervisor, doctor and carries out prescribed medical and nursing procedure.

8. Operates or supervises the operation of specialized equipment assigned to unit and provides assistance and guidance to nursing team as required.

9. Assists new patients/residents in adjusting to their new environment and informs them of facility rules and regulations.

10. Prepares patient/resident and family for potential discharge.

11. Responsible for carrying out fire and safety regulations to maintain continuing safe environments for patient/residents

12. Facilitates Nursing Instructions for CNAs to ensure patient safety and quality care.

13. Assure infection control standards such as universal precautions, contract precautions, droplet precaution and hand washing protocols are used.

14. Prevent resident elopement and injury through identification of those at risk and implementation of measures to promote their safety.

15. Ensures that report is given to on-coming nurse on conditions of patient/residents and/or any unusual problems and action taken.

16. Exhibits sensitivity to and awareness of the special needs of resident/patients with Dementia and the needs of the subacute and terminally ill patient/resident. Completes required documentation daily.

17. Follows code of conduct and ensures resident privacy and is compliant with HIPAA.

18. Will perform other related duties as assigned.

Required Qualifications, Skills and Abilities

1. Education: Completion of a formal education program in Nursing

2. License/Certification: Valid NYS LPN License, CPR certified

3. Equipment/Computer Software: Basic computer/ EMR software

4. Physical/Mental Demands: Walk long distances, climb stairs, run in an emergency and remain on her / his feet for long durations of time. Bend, squat, push, pull, reach, stretch and lift up to 50lbs. Speak, read and understand English. Hear the spoken word. Demonstrate hand/eye dexterity for operating ADL and medically complex equipment, e.g. oxygen concentrator, I.V. equipment, parenteral feeding equipment, peritoneal dialysis equipment, ventilator equipment, etc.
